مطالعه روشهای وقايه از انتانات پس از عملیات جراحی در سرویس جراحی عمومی شفاخانه حوزوی هرات از اول ماه جدی 1404 الی آخر ثور 1405

زمینه و هدف: انتانات محل جراحی قابل پیشگیری هستند، اما هنوز هم باعث عوارض قابل توجه، میزان مرگ و میر بالا و فشار مالی بر بیماران میشوند. استفاده نامناسب از پروفیلاکسی ضدمیکروبی جراحی در حال افزایش است و کیفیت زندگی بیماران را بدتر میکند.
روش: جهت جمعآوری ارقام و منابع تحقیق از دوسیه مریضان و راپور HMIS گرد آوری شد. از میان مریضان مذکور، تعداد 79 مریض بصورت تصادفی انتخاب شدند. تحقیق از نوع توصیفی- مقطعی با روش کمّی و بهصورت گذشتهنگر طراحی گردیده است.
یافتهها: بیشترین واقعات مربوط به گروه سنی ۵۱ تا ۶۵ سال با ۲۸ بیمار (٪۳۵.۴) و کمترین آن مربوط به گروه بالای ۸۰ سال با ۱ بیمار (٪1.3) بود. توزیع جنسیتی تقریباً مساوی بود، ۴۰ مرد (٪۵۰.۶) و ۳۹ زنان (٪۴۹.۴). از نظر بروز انتانات محل جراحی، ۱۱ بیمار (٪13.9) دچار انتانات شدند و ۶۸ بیمار (٪۸۶.۱) بدون انتانات بودند. بیماریهای مزمن (مانند دیابت و بیماری کلیوی) با ۳۶ بیمار (٪۴۵.۶) شایعترین عامل خطر بود؛ پس از آن چاقی (۲۷ بیمار، ٪۳۴.۲)، استعمال دخانیات (۱۸ بیمار، ٪۲۲.۸) و سوءتغذیه (۱۵ بیمار، ٪۱۹) قرار داشتند. از نظر نوع زخم جراحی، زخمهای تمیز (۲۹ بیمار، ٪۳۶.۷) و تمیز-آلوده (۲۷ بیمار، ٪34.2) بیشترین سهم را داشتند. سایر عوامل نشان داد که ۳۳ بیمار (٪۴۱.۸) سیستم ایمنی تضعیفشده، ۲۹ بیمار (٪۳۶.۷) در شرایط اتاق عمل نیازمند بهبود، و ۴۰ بیمار (٪۵۰.۶) مدت بستری پیش از عمل بیش از ۲۴ ساعت داشتند.
نتیجهگیری: مهمترین عوامل خطر مربوط به بیماریهای مزمن و نیز عوامل قابل اصلاح مانندچاقی، سوءتغذیه و استعمال دخانیات بود. همچنین ضعف ایمنی، وضعیت ناکافی اتاق عمل و طولانی شدن بستری پیش از عمل با افزایش بروز انتانات همراه است؛ لذا تمرکز بر برنامه پیشگیری، به همراه کنترل عوامل بومی و بهینهسازی شرایط محیطی توصیه میشود.
